A Journey to healing: successful limb salvage and functional restoration in chronic femoral nonunion with osteomyelitis via Ilizarov fixation

Abstract

This report documents the healing process of a 20-year-old male patient who faced a major orthopedic challenge through a transverse femoral shaft fracture with substantial translation after initial interlocking intramedullary nailing resulted in persistent nonunion and chronic osteomyelitis. A severe injury transformed into an intricate battle that involved infection alongside bone degeneration and a significant discrepancy in limb length. This report outlines the transformative implementation of the Ilizarov method as a final solution which uses specialized external fixation techniques. The Ilizarov treatment system combines controlled bone regeneration with multiplanar stability to produce complete bone union and eliminate persistent infections while restoring the natural length and alignment of the limb. The patient’s experience ended with complete healing and zero deformities or length discrepancies while achieving full mobility. This case demonstrates the exceptional capability of the Ilizarov method to rehabilitate limbs and restore function in complex orthopedic situations that would otherwise require amputation. This patients’ journey starts in October 2023 with complications and ends with a complete cure in June 2025 by the advancement treatment procedure in Bari- Ilizarov Orthopedic Centre.
