Successful limb salvage in metastatic proximal humerus osteosarcoma: biological reconstruction with free fibular autograft and Ilizarov osteosynthesis following intensive neoadjuvant therapy-a case report

Abstract

Context: Osteosarcoma in the upper arm, right near the shoulder, is tough to deal with. It’s a fast-moving, dangerous bone cancer, and the location alone makes surgery tricky. Things get even more complicated when the cancer spreads. Around 68% of people make it five years if the cancer stays put, but that drops quickly once it spreads. To really save the arm, to go after the cancer with strong treatments before even think about surgery.1,2 Clinical summary: A 20-year-old male, otherwise healthy, ended up with Stage III/IV osteosarcoma in his right upper arm. He went through two years of intense chemotherapy with several drugs, plus radiation. It wasn’t a short or easy road, but the results speak for themselves-his distant metastases have stayed stable and under control.3 Surgical intervention: The patient had a large section of their upper humerus removed because of the tumor- about 15 centimeters, which is no small gap. To rebuild the bone, use a piece of the patient’s own fibula, taking the head and the upper part of the shaft. Here it’s not use microvascular surgery for this; instead, fixed the graft in place with a hybrid Ilizarov external fixator, using dynamic tension to help the bone heal. The frame stayed on for three months.4 Outcome: With this approach-combining several therapies and using the patient’s own bone for reconstruction-they got full bone healing on X-rays during the time the fixator was on. The patient kept their limb and didn’t need an amputation. When it measured function with the MSTS score, the results were excellent. This shows that, even in tough cases with lots of bone loss, this demanding technique can actually bring solid, lasting results.4,5
