Abstract
Hemodialysis treatment requires continuous nursing care addressing clinical, educational, and psychosocial needs. This study aimed to develop a Nursing Care Plan for adults undergoing hemodialysis, grounded in scientific evidence and structured according to the Nursing Process. A systematized literature review with structured narrative synthesis was conducted. Of the 1,528 records identified, 24 studies were included in the final synthesis. The evidence focused on health education and self-care; vascular access management and preservation; prevention and management of complications and symptoms; and psychosocial support and continuity of care. The findings supported the development of the Nursing Care Plan according to the stages of the Nursing Process. The proposed plan requires further validation and assessment of its clinical applicability.
